The vast majority of our new patients are invited to our practice by their family, friends and coworkers. We consider these invitations to be the highest compliment a patient can give to us. Regardless of how you came here, we feel incredibly blessed that you have chosen us to provide your dental care. What To Expect
Upon contacting our office for a new patient examination, our staff will conduct a brief interview to get some basic information about you as well as to discover your particular areas of concern. You will then receive a packet from our office containing all the necessary forms for you to complete prior to your first visit. We ask that you take a few moments to fully answer the medical and dental history forms as completely as possible giving as much detail as possible.
In addition, we ask that you fill out the dental records release form and send or deliver it to your previous dental care provider so that your previous records can be forwarded to us. It is best if we receive your old records in advance of your appointment to allow us time to research your care. If that is not possible, we will thoroughly review them after your appointment and prior to finalizing any treatment recommendations.
Please arrive 15 minutes prior to your scheduled new patient appointment time with the completed paperwork and any old records you were able to obtain in person. If you forgot or were unable to fill out the paperwork in advance, please arrive at least 30 minutes in advance. Our staff will be happy to greet you and will complete the registration process. Please provide your current dental benefit/insurance card should you have one as well as your drivers license for verification purposes.
Once registered, you will have the opportunity to meet the staff and take a brief tour of our office. We will review your medical and dental histories along with any available dental records. We will discuss your dental health concerns and learn your healthcare objectives. A comprehensive radiographic and clinical head and neck exam will then be performed evaluating you teeth, jaws, temporomandibulat joints, bite relationship and periodontal health. We will also screen for head and neck cancer as well as other soft tissue diseases.
After the examination is complete, we will summarize the findings in easy to understand terms and give you a chance to ask any questions you may have. A thorough problem list will be fabricated as well as a list of treatment options available to address those issues and to satisfy your short and long-term dental health goals. We will then partner with you to match your treatment options to your desires and budget.
Once you have selected a plan that is appropriate for you, we will summarize this plan in written format that lays out the specific treatment, sequence order, appropriate fees and an estimate of your out-of-pocket expenses. You will then be given the opportunity to review the plan and schedule an appointment to begin the process of restoring and/or maintaining your oral health.
Our office is conveniently located in the Mill Square Professional building one-half block west of downtown Armada. You will find Suite 200 in a park-like setting at the rear of the building overlooking beautiful Coon Creek.
